If you would like to have a phone consultation prior to making an appointment, I am available to answer any questions you may have regarding your health concerns and if you have a condition that I may be able to help.
Free Consultations
We offer free consultations if you would like to meet me and help you determine if I am a good fit for serving your healthcare needs. An appointment for a complimentary consultation lasts approximately 15 minutes.
Your Initial Visit
New patients are required to complete a few forms prior to the initial visit. This is necessary so I have a thorough understanding of your current and past health history. You have the option of completing these forms in the comfort of your own home and bringing them to our clinic or completing them at our clinic when you arrive. If you would like to complete the forms at our clinic, we ask that you arrive at least 30 minutes prior to your appointment time.
After completing your forms, we will review your health history, perform a thorough examination, and take any necessary x-rays to determine the cause of your condition.
Your initial visit will last approximately 1 to 1.5 hours.
Insurance
Blue Cross Blue Shield PPO and Medicare Part B are the only insurances I accept. I still accept patients that have other insurance. If you have any other insurance or a Health Savings Account, you will be considered a self-pay patient and we can provide you with a receipt if you would like to submit to your insurance company for reimbursement.
